I highly recommend completing the prompts on page 3 and 4. Hit the ones that jump out at you, but also take a look at any prompts that you're resistant to. Begin where the resistance is. If it's something you've never thought of or something you don't have an answer to... just begin writing and see what you uncover. Notice what opens up for you. These relabel saboteurs (complaints + sufferings) are your signposts to healing.
